How to register
We ask that you complete BOTH of the forms below because it can sometimes take considerable time for us to receive your medical history from your previous practice.
Acceptance on to the list is as at the discretion of the Doctor and we may ask you for proof of identity. (Please refer to our registration policy below to find out why we ask for this).
Accountable named GP for all patients
All patients registered at Stockport Medical Group have a named doctor who has overall responsibility for your care and support. Your registered GP is also your named accountable and your allocated GP. Your allocated GP will be responsible for the provision of your healthcare. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.
If you are unsure of who is your named GP please ask at reception.
New patients are allocated a Doctor on Registration.
Find your NHS number
Your NHS number is a 10 digit number, like 485 777 3456. You do not need to know your NHS number to use NHS services, but it can be useful to have it.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
