START – Stockport Triage Assessment Referral Team

START is the main point of contact to access Stockport lifestyle change services. Lifestyles covered are weight management, stop smoking, physical activity, PARiS, alcohol support and drug support.

For those wishing to improve their health through improving their lifestyle. Residents of Stockport SK1-SK8 or those registered with a Stockport GP.

  • Alcohol and drug support is for those 26 yrs old and over

For those wishing to improve their health through improving their lifestyle. Residents of Stockport SK1-SK8, those registered with a Stockport GP or working in Stockport

  • Weight management is for 18 yrs and over
  • Stop smoking is for 12 yrs and over
  • Physical activity 18 yrs and over
  • PARiS 18 yrs and over

The patient can be signposted if the patient is otherwise well and:

Quit smoking above age of 12 and above – For those with a Stockport address SK1 – SK8 or Stockport GP or working in Stockport. Low mood, financial difficulties, isolation and anxiety as a result of caring responsibilities.

Weight management 12 week programme for  age 18 and above with a BMI of 25 and over- For those with a Stockport address SK1 – SK8 or Stockport GP or working in Stockport

(Specialist) Physical Activity Referral in Stockport – PARiS Age 18 and above – For those with a Stockport address SK1 – SK8 or Stockport GP or working in Stockport

Alcohol support for those 26 years old and above – For those with a Stockport address SK1 – SK8 and/or Stockport GP

Drug misuse for those 26 years old and above – For those with a Stockport address SK1 – SK8 and/or Stockport GP

The patient CANNOT be signposted if:

Under age of 12

Not for those with BMI under 25

Not for those under age of 18

Under 26 years old – START is NOT an emergency service and does not prescribe.

START respond to online (website) referrals within 48 hrs and respond immediately to referral calls into the office.
START offer 2 approaches depending on the type of request received.

Telephone Triage for those seeking advice/support with Stop Smoking, Weight Management & Healthy Eating and/or Physical Activity. After telephone triage, the client is referred onto the most appropriate service/s to assist with lifestyle change. START transfer the patients information and the service will contact the patient.

1-1 Assessments for ALL alcohol and drug referrals. Once assessment is completed START refer onto the most appropriate specialist treatment service in Stockport where the person will receive their treatment plan.
