How to register

Acceptance on to the list is as at the discretion of the Doctor and we may ask you for proof of identity. Please find out why we may ask for this information or view our Registration Policy

If you are unable to register online then please find the paper form which you can download and complete, or you can collect from the Practice.

All patients registered at Stockport Medical Group have a named doctor who has overall responsibility for your care and support. Your registered GP is also your named accountable and your allocated GP. Your allocated GP will be responsible for the provision of your healthcare. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.

If you are unsure of who is your named GP please ask at reception.

New patients are allocated a Doctor on Registration.

Your NHS number is a 10 digit number, like 485 777 3456. You do not need to know your NHS number to use NHS services, but it can be useful to have it.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
